Urgent Help - Senior w/dementia?

Short Story: my mom and dad are living in their home together.
- Dad has some health issues, but can definitely live on his own in an apartment.
- unfortunately, mom's memory issues have become severe. She really needs to be in an assisted living facility because she cannot really take care of herself even though she can do things like feed, go to the bathroom/shower, dress ... herself.
- we have taken away the keys to the cars.
- her issue is she cannot manage her doctors or her medication. Example, about 4 weeks ago, she nearly died from an accidental overdose of blood pressure medication.
- because of her memory issues, she has verbal outburst that aren't quite "abuse" but she is making dad miserable.

Bottom Line: I don't think dad can take care of mom anymore. Furthermore, he is compromising his health trying to do so.

Question: if my dad leaves the house, legally what happens? We know she should not live by herself, but she is probably ok to live in the house with an aid (which she will not accept)

Note: there is money available to put her in a nice assisted living facility.
