in my opinion
1. By far, the offense fumbles the ball and it goes out of the end zone and it's the defense's ball on the 20. Give the fumbling team the ball at the point of the fumble and be done with it.
2. The offense makes a substitution, with plenty of time left and the defense can lollygag a player in causing a delay of game or force a TO. Not 100% sure how to correct.
3. Being "covered up" making a player an ineligible receiver. Seems like a rule that lost its relevance when we got rid of leather helmets
